Help for Injured Workers

If you have suffered an injury at work, please notify your employer immediately and seek treatment as soon as possible.

 

Call 911 for all emergency situations or go to the nearest emergency room

 

Your employer will submit a claim with Preferred Employers Insurance and provide you with a claim form to complete. Complete only the “employee” section. Be sure to sign and date the claim form and keep a copy for your records. Return the claim form to your employer right away in person or by mail, a delay in returning this form may risk your benefits.

MPN Notice

Should you need to seek additional medical treatment, Preferred has directly contracted with medical providers throughout the State of California in their Preferred Select Medical Provider Network (MPN). All treatment is to be provided by a participating provider in the MPN. Please refer to MPN Notice for more helpful information about scheduling and obtaining care in the MPN.

Treatment Authorization Form

To expedite your first medical appointment, please bring a completed Treatment Authorization form with you. This form also includes a first fill pharmacy card to obtain prescription medication if needed.

Electronic Payments

Preferred offers electronic payment directly to your checking or savings account. To initiate this process, complete the EFT Request form and email to [email protected]. A member of our team will reach out to you to confirm you bank details and complete the setup.

Travel Repayment

If you have to travel to get treatment for your work injury, you are entitled to repayment of your travel costs. Mileage for reasonable travel to the pharmacy, parking, bridge tolls, public transportation and other travel-related costs are also included. Complete the Mileage form, attach receipts and send to:

Utilization Review

Medical providers are to submit all Request For Authorization (RFA) directly to Preferred’s Utilization Review department via fax at (866) 921-7313. Please include all relevant supporting medical reports when submitting requests for review.

 

If the physician reviewer is unable to recommend approval, you will be notified and the have the opportunity to discuss the request with Preferred reviewer and provide additional information as needed thru a voluntary appeal process.

 

Requests For Authorization’s not received on Preferred’s dedicated UR fax line are considered invalid and will not be processed.

Medical Provider Network (MPN)

Our team works closely with injured workers to match them with an MPN physician that is conveniently located near their place of residence. All of our medical providers are annually qualified and directly contracted with our Medical Provider Network (Preferred Select MPN MPN log #1226).
